In our continued efforts to provide a safe and confidential platform for our staff members and volunteers to report activities and incidents that warrants the attention fo the Élan Youth Arts Board and Administration, we are providing the following Incident Report for your use to report these activities and incidents.

The Incident Report will serve as a record for the Élan Youth Arts Board and Administration for all matters in which staff members and/or volunteers are either witness to or involved with matters pertaining to safety of any individual affiliated with the Élan Youth Arts organization.
